Remote Therapeutic Monitoring (RTM) Program — Patient Consent
What is Remote Therapeutic Monitoring?
Remote Therapeutic Monitoring allows the ROCF care team to monitor recovery between office visits using brief daily text-message check-ins. Patients receive a short daily message about pain, function, and symptoms, and responses are routed back into the ROCF workflow.
How it works
- The patient receives a brief daily text message
- The patient responds in about 60 seconds
- The care team reviews responses and follows up if needed
- Patients can text back with concerns
What this program costs
There is no cost to the patient. RTM is billed through standard care workflows as part of ongoing orthopaedic care.
Privacy
- Communications are handled through secure systems
- Patient information is protected within ROCF's care workflow
- Information is not sold or used for marketing
- Only authorized care team members access the data
CONSENT
☐ Text message communication: I agree to receive text messages from Reconstructive Orthopaedics of Central Florida related to medical care and recovery monitoring. I understand I can opt out at any time by replying STOP.
☐ Remote Therapeutic Monitoring: I understand that my care team will use my check-in responses to monitor recovery. I understand this does not replace emergency care.
☐ De-identified research use (optional): I voluntarily agree that de-identified health data may be used for research purposes to improve patient care.

I wanted to give you a heads-up on a new program we're rolling out at ROCF: Remote Therapeutic Monitoring (RTM).
What it is: Patients get a daily text asking about pain, function, and recovery. Staff monitors the dashboard. If something looks off, it gets flagged for review.
Why we're doing it:
- Better patient communication
- Earlier visibility into recovery problems
- A reimbursable care workflow
What changes for you: Very little operationally. Staff handles enrollment and monitoring; providers get pulled in when a patient needs clinical review.

Key rules: one 30-day billing period, one provider per period, one live interaction per period for management codes, and no RTM/RPM overlap in the same month.
