What is Remote Therapeutic Monitoring?

Remote Therapeutic Monitoring (RTM) is a program that allows your care team at ROCF to monitor your recovery between office visits using brief daily text message check-ins. Each day, you will receive a text message asking about your pain level, function, and how you are feeling. Your responses go directly to your surgeon's care team โ€” not a call center or answering service.

How It Works

  1. You will receive a daily text message with a few brief questions about your recovery
  2. You respond by text โ€” it takes about 60 seconds
  3. Your care team reviews your responses and reaches out if anything needs attention
  4. If you have a concern, you can text back anytime โ€” your message goes straight to the practice

What This Program Costs You

There is no cost to you. RTM is billed to your insurance as part of your ongoing care. You will not receive a separate bill for this service.

Your Privacy

  • All communications are private and HIPAA-secured
  • Your health information is transmitted through encrypted, secure channels
  • Your information will never be sold, shared, or used for commercial or marketing purposes
  • Only your care team at ROCF has access to your responses

โ˜ Remote Therapeutic Monitoring: I understand that my care team will use my daily check-in responses to monitor my recovery. I understand this does not replace emergency care โ€” if I experience a medical emergency, I should call 911.

โ˜ De-Identified Research Use (OPTIONAL): I voluntarily agree that my de-identified health data (with all personal information removed) may be used for medical research purposes to improve patient care. This is entirely optional and does not affect my treatment.

I wanted to give you a heads-up on a new program we're rolling out at ROCF: Remote Therapeutic Monitoring (RTM).

What it is:
Patients get a daily text message asking about their pain, function, and recovery. Takes them 60 seconds. Their responses come into a dashboard that our staff monitors. If something looks off, the system flags it and we get notified. If everything's on track, the patient gets an encouraging response automatically.

Why we're doing it:

  1. Better patient communication โ€” patients get a direct line back to the practice every day, no phone tag
  2. Catch problems earlier โ€” we see pain spikes, wound concerns, and compliance issues before the next office visit
  3. It's reimbursable โ€” CMS has specific CPT codes for RTM (98975-98981, 98984-98985). Conservative estimate is $400-640 per patient per 30-day period

What changes for you:
Honestly, very little. The system runs in the background. Staff handles enrollment at checkout and monitors the dashboard daily. You'll only hear about it when a patient gets flagged for clinical review โ€” and that's information you'd want anyway.

Who's eligible:
Any MSK patient โ€” post-op (ORIF, TKA, THA, rotator cuff, etc.) AND nonoperative (fractures in a sling, knee OA, chronic MSK). It's broader than you'd think.

What I need from you:
Nothing right now. I'm pilot-testing with a few of my patients first to work out any kinks. Once it's running smoothly, we'll open it up to all providers. Just wanted you to know it's coming so there are no surprises.

We're launching a Remote Therapeutic Monitoring (RTM) program at ROCF and I want to walk you through the billing side so we're all on the same page.

What RTM is:
Patients receive daily text message check-ins about their recovery (pain, function, medication). Their responses are monitored by our staff through a dashboard. This is a CMS-recognized service with dedicated CPT codes.

Key billing rules:

  • Billing period = 30 calendar days from enrollment start
  • 98975 (setup) is billed ONLY in the first period
  • 98976 (device supply) is billed every period
  • Choose 98985 OR 98980 for management โ€” never both in the same period
  • 98981 is an add-on to 98980 only (not 98985)
  • One live interaction (phone or video call) required per billing period for management codes
  • One provider per patient per 30-day period
  • RTM and RPM cannot be billed for the same patient in the same month
  • General supervision is sufficient

What you'll receive from us:
Our system will generate a billing export at the end of each 30-day period with patient name, DOB, insurance info, billing provider (NPI), diagnosis code (ICD-10), applicable CPT codes (auto-calculated), documentation (monitoring days, management minutes, live interaction date), and period dates. We can deliver as CSV or whatever format works best for your workflow.

Revenue potential:
A typical patient with 16+ monitoring days generates $124-166 per 30-day period. With even 10-15 active patients, this is a meaningful new revenue line for the practice.

Payer coverage:
Medicare covers RTM. Commercial: Aetna, Cigna, Florida Blue, several BCBS plans, Humana, and Elevance Health have RTM coverage. UHC varies by plan. We'll track denials and adjust as we learn which payers are paying cleanly.

I'm pilot-testing with a small group of patients first. You'll start seeing the first billing exports in about 5-6 weeks. Happy to jump on a quick call if you want to walk through anything.
