Use of ROCFcare

1. Acceptance of Terms

By accessing or using ROCFcare, you agree to these Terms of Service. The service is operated by Reconstructive Orthopaedics of Central Florida (“ROCF”). If you do not agree, do not use the service.

2. Description of Service

ROCFcare is a Remote Therapeutic Monitoring platform that allows patients to report symptoms and communicate with the orthopaedic care team. Features may include:

• Daily check-in forms for pain, function, and symptoms
• AI-assisted triage support
• Secure patient-to-care-team communication
• Recovery tracking and workflow support

4. AI Disclaimer

• AI is used to assist screening and prioritization
• AI does not diagnose or prescribe treatment
• Clinical review remains the governing standard
• AI outputs may be imperfect and should not replace professional evaluation

5. Patient Responsibilities

• Provide accurate information during check-ins
• Report urgent or worsening symptoms promptly
• Keep contact information current
• Protect any account credentials provided
• Continue regular appointments and follow clinical instructions

6. Staff Responsibilities

• Access only the information necessary for their role
• Maintain confidentiality of patient information
• Protect credentials and report security concerns
• Follow applicable privacy and HIPAA requirements

7. Cost

There is no cost to patients for using ROCFcare. RTM services may be billed through standard care workflows, but patients are not charged by ROCFcare as a standalone platform fee.

8. SMS Text Messaging & Opt Out

ROCFcare uses automated, non-promotional SMS text messages to deliver recovery check-ins, reminders, care coordination updates, and secure links to complete recovery monitoring tasks. By enrolling and checking the consent box on the enrollment form, you agree to receive these messages during an active episode of care.

• Message frequency: Varies; approximately 1 message per day during an active monitoring episode
• Msg&Data rates may apply depending on your wireless carrier and plan
• To opt out: Reply STOP to any ROCFcare message. You will receive a confirmation and no further messages will be sent unless you opt back in. You may also call (352) 456-0220

• To re-enroll: Reply START or YES after opting out
• Consent is not required as a condition of purchase or receiving treatment from ROCF
• Consent sharing: SMS consent is not shared with third parties or affiliates for marketing purposes

Opting out of text messages does not end the underlying orthopaedic care relationship. You may continue follow-up care through regular office visits.

9. Availability

ROCFcare is intended to be available consistently, but uninterrupted access cannot be guaranteed. Availability may be affected by maintenance, updates, connectivity issues, or external service dependencies.

10. Limitation of Liability

To the maximum extent allowed by law, ROCF is not liable for indirect, incidental, or consequential damages arising from use of the service. The platform is provided as part of care operations subject to applicable legal requirements.

11. Governing Law

These terms are governed by the laws of the State of Florida. Disputes related to the service are subject to applicable Florida jurisdiction and venue requirements.

12. Changes to Terms

ROCF may update these terms from time to time. Continued use of the service after updates are posted constitutes acceptance of the revised terms.
